Custom medication transfer integration development
Appfront builds integrations for medication exchange: the digital exchange of medication data between prescribers, pharmacies, care organisations and the patient. Under the Nictiz information standard Medicatieproces 9, prescriptions, dispensings and administrations are shared between systems, so everyone works from the same up-to-date overview. Medication exchange is one of the exchanges regulated under the Wegiz.
What is an integration for medication exchange?
In medication transfer, medication data moves digitally between the people and systems that work with it: the doctor who prescribes, the pharmacy that dispenses, the nursing staff who administer, and the patient who takes the medicine. The Nictiz information standard Medication Process 9 describes these four steps and distinguishes between agreements about treatment and the dispensing of the medicine. This creates an overview that is the same for everyone.
Medication transfer is one of the data exchanges regulated under the Dutch Electronic Data Exchange in Healthcare Act, the Wegiz. Later regulations set requirements for how the exchange is carried out digitally, based on the standard. To find out which requirements apply to your organisation, follow the announcements from the government and Nictiz; if in doubt, check with your adviser.
In practice, medication data is still often sent by fax, PDF, or retyped from one system into another. A change made when a patient is discharged from hospital doesn't reach the administration list in the nursing home. We build custom development that lets your system exchange data in line with the standard, connected to the infrastructure your chain partners use.
According to the standard
Agreements, dispensing and administration according to Medication Process 9, to and from your own system.
Connecting the chain
Prescriber, pharmacy, care organisation and patient work from the same up-to-date overview.
Prepared for the Wegiz
An integration that meets the requirements set for medication exchange under the Wegiz.
How we build your medication exchange integration
We start with your system and chain: which medication data you hold, which partners you want to exchange with, which infrastructure they use, and which version of the standard. Your own data is often stored differently from how the standard describes it. We work out that translation carefully, together with the people who work with the medication.
Your data, partners, infrastructure and version of the standard.
Your data to and from the building blocks of Medication Process 9.
Testing with partners, and the qualification that applies to the standard.
Go-live, monitoring, and maintenance when new versions are released.
What a medication exchange integration does in practice
The components below feature in almost every medication exchange integration. Which ones you need depends on your role in the chain.
Agreements
Medication agreements and administration agreements.
Dispensing
What the pharmacy has dispensed.
Administration
What has been administered, and what has not.
Use case
Medication use as reported by the patient.
Legal basis
Consent and legal basis recorded.
Audit log
Every message logged, including errors.
Who we build medication exchange integrations for
The integration is intended for anyone who exchanges medication data within the chain.
Software suppliers
A product for healthcare. The standard is at its core.
Hospitals
Admission and discharge. The transfer matters most.
Long-term care
Administration by nursing staff. The administration agreement is what is needed.
Pharmacies
Dispensing to many organisations. The agreements are at the core.
Technology and integrations
This page is about medication exchange. For pharmacies, see our page on pharmacy software; for healthcare in general, our page on healthcare software; and for referrals, our page on a ZorgDomein integration. Read about our way of working at building software.
For the approach to all designated exchanges together, including certification, see our integration for the Wegiz.
For the wider set of data required for each handover, see our page on a BgZ integration.
Why choose Appfront for your medication handover integration?
A medication change that does not arrive can harm a patient. That is what we build around: data that follows the standard, a chain that shares the same overview, and an integration that fits what the Wegiz requires.
Safer
Everyone sees the same up-to-date medication.
Less retyping
No fax or PDF between systems.
Ready for the law
Follows the Wegiz standard.
Security and privacy in a medication handover integration
The integration processes medical data, which is particularly sensitive. Only data with a legal basis or consent is exchanged, connections are encrypted, and every message is logged. What is permitted, you decide together with your data protection officer.
The integration runs in a European data centre or in your own environment, with encrypted storage, daily backups and strict role-based access.
Frequently asked questions about a medication handover integration
Questions care organisations ask before getting started.
The digital exchange of medication data between prescribers, pharmacies, care providers and the patient, so that everyone works from the same up-to-date overview. It is one of the data exchanges regulated under the Wegiz.
Nictiz's information standard for recording and exchanging medication data. It describes prescribing, dispensing, administration and use, and distinguishes between the agreements about treatment and the supply of the medicine.
The requirements for medication handover are being set out in regulation in stages. For when which requirements apply to your organisation, follow the messages from the government and Nictiz, and align with your adviser or industry body.
With the systems of your chain partners that support the standard, via the infrastructure used in your chain or region. Which route fits is looked at in the first step.
We map how your system records medication and translate that into the building blocks of the standard, such as medication agreement, dispensing and administration. Where your data is structured differently, we decide together with the people who work with the medication how it should be handled.
The integration checks messages before sending and on receipt. An error is reported with its cause, and a received message that cannot be processed is set aside for review, so no data is lost unnoticed.
For care software vendors who need to support the standard in their product, and for healthcare organisations with their own systems, such as an administration registration or an in-house record, that do not connect through a standard package.
